Abdominoplasty is one of the most lymphatically and structurally significant procedures in plastic surgery. The procedure elevates a large abdominal skin flap, disrupts lymphatic vessels across a broad territory, and repositions or tightens the abdominal musculature — producing extensive swelling, fluid accumulation, and tissue changes that require dedicated, skilled rehabilitation to resolve safely and efficiently.
A specific concern after abdominoplasty is seroma formation — pooling of lymphatic fluid in the space beneath the elevated skin flap. Manual Lymphatic Drainage after drain removal is one of the most effective strategies for preventing seroma by actively facilitating fluid clearance from this space. The horizontal scar also requires careful management to prevent adhesion, tightening, and the characteristic tissue shelf that can develop above the incision line.
Drain status: We do not begin direct abdominal MLD while surgical drains are in place unless specifically directed by your surgeon. Once drains are removed and clearance is given, MLD becomes an important tool for preventing the fluid accumulation drains were managing.
Post-Abdominoplasty Concerns We Treat
- Abdominal swelling and heaviness persisting weeks after surgery
- Seroma risk and prevention after drain removal
- Firmness and induration in the abdominal skin flap
- Fluid migration to the pubic area, inner thighs, or flanks
- Tightness and restricted trunk movement from swelling and scar tension
- Horizontal scar management — preventing adhesion, shelf formation, and tightness
- Numbness and hypersensitivity at the incision line
- Core reconnection guidance and return-to-activity planning
Our Treatment Approach
Staging Assessment
We review your drain status, incision healing, current swelling, and surgeon's guidelines before beginning. Early sessions focus on proximal lymph node basins before advancing to the abdominal flap as healing allows.
Abdominal MLD
Gentle MLD along inguinal and abdominal drainage pathways facilitates fluid clearance from the flap and reduces seroma risk after drain removal. Sessions are adapted continuously to your healing stage. Typically 60 minutes.
Scar & Fibrosis Management
Once the incision has adequate healing, scar desensitization and mobilization are incorporated — addressing the shelf, numbness, and tissue adherence at the horizontal scar line. See our dedicated scar management services.
Mobility & Core Guidance
Positioning advice, gentle movement progression, and basic core reconnection guidance to support functional recovery alongside lymphatic and scar care.
Abdominoplasty recovery is significantly more comfortable and efficient with certified rehabilitation. Free consultations available.

Frequently Asked Questions
When can I start rehabilitation after my tummy tuck?
Most patients begin once drains are removed and the incision has adequate closure — typically 1–3 weeks post-operatively. We always confirm timing with your surgeon before beginning abdominal work.
I had a tummy tuck combined with liposuction. Does that change the protocol?
Yes — combined procedures increase the extent of lymphatic disruption and the total area requiring drainage. We address all affected regions in a coordinated sequence and may recommend more frequent initial sessions.
Can rehabilitation help with the tissue shelf above my scar?
Yes. The shelf is caused by a combination of fluid accumulation and scar tissue adherence. MLD addresses the fluid component; scar mobilization addresses the tissue adherence. Together these significantly reduce the shelf and improve tissue pliability.
