Abdominoplasty is one of the most lymphatically disruptive procedures in plastic surgery. The procedure involves elevating a large flap of abdominal skin and fat, repositioning or tightening the abdominal muscles, and excising excess tissue — all of which transect numerous lymphatic vessels and capillaries across a broad anatomical territory. The resulting disruption to lymphatic drainage in the abdomen, flanks, and lower trunk produces significant, often prolonged swelling that requires skilled intervention to resolve efficiently.
A further concern specific to abdominoplasty is seroma formation — the accumulation of lymphatic fluid in the space between the elevated skin flap and the underlying muscle fascia. While surgical drains manage early seroma risk, the period after drain removal is when MLD becomes particularly valuable: by actively stimulating remaining lymphatic pathways, MLD reduces the fluid accumulation that would otherwise pool and potentially require aspiration.
Our CLT-certified therapists are trained in the specific lymphatic anatomy relevant to abdominoplasty recovery and adapt their approach to the post-operative timeline, drain status, and your surgeon's instructions at every stage of treatment.
Drain management: We do not begin abdominal MLD while surgical drains are in place unless specifically directed by your surgeon. Once drains are removed and clearance is given, MLD becomes an important tool for preventing the fluid accumulation that drains were managing.
Post-Abdominoplasty Concerns We Treat
- Abdominal swelling and heaviness that persists weeks after surgery
- Risk of seroma formation after drain removal
- Firmness and induration in the abdominal skin flap
- Fluid migration to the pubic area, inner thighs, or flanks due to gravity
- Tightness and restricted trunk movement from swelling and scar tension
- Bruising and discoloration slow to resolve
- Discomfort with sitting, walking, or changing positions
Our Post-Abdominoplasty MLD Protocol
Staging Assessment
We assess drain status, incision healing, current swelling distribution, and your surgeon's post-operative guidelines before beginning any abdominal work. Early sessions may focus on proximal lymph node basins (axillary, inguinal) before addressing the abdominal flap directly, as the tissue heals and tolerates contact.
Manual Lymphatic Drainage
Gentle MLD works systematically from the lymph node basins outward, creating a drainage pathway before stimulating fluid movement from the congested abdominal tissue. Technique and pressure are adapted continuously to your healing stage and tissue response. Sessions are typically 60 minutes.
Scar & Fibrosis Management
As healing progresses, we incorporate scar mobilization at the horizontal incision line and any areas of developing fibrotic tissue in the abdominal flap. This supports pliable, mobile tissue and helps prevent the hardening that can occur when lymphatic drainage is prolonged. See our scar management services.
Compression & Activity Guidance
We coordinate with your surgeon's compression garment protocol and advise on positioning, gentle movement, and activity modification to support lymphatic recovery between sessions. A simple self-drainage routine is taught for use at home.

Tummy Tuck MLD — FAQ
When can I start MLD after my tummy tuck?
Timing depends on whether you have drains in place and your surgeon's guidelines. Most patients begin once drains are removed and the incision has adequate closure — typically 1–3 weeks post-operatively. We always confirm timing and approach with your surgical team before starting abdominal work.
Can MLD help prevent seroma?
MLD supports the clearance of lymphatic fluid from the space beneath the abdominal flap, which is the fluid that would otherwise accumulate as a seroma. While MLD cannot guarantee seroma prevention, it reduces the fluid load significantly and complements the role drains play in early recovery. Many surgeons specifically recommend MLD after drain removal for this reason.
I had a tummy tuck combined with liposuction. Does that affect the protocol?
Combined procedures increase both the extent of lymphatic disruption and the total area requiring drainage. We address all affected regions in a coordinated sequence and may recommend more frequent initial sessions. Your procedure details are reviewed at the first visit and the plan is adapted accordingly.
