Total (Simple) Mastectomy & Why Rehabilitation Matters

Total mastectomy removes the entire breast including the nipple-areola complex and overlying skin, while preserving the chest wall muscles. It is the most common mastectomy type and is frequently followed by immediate or delayed breast reconstruction. Rehabilitation addresses the chest wall changes, axillary findings from sentinel node biopsy, scar management, and the lymphedema risk created by axillary disruption.

Part of our complete breast rehabilitation program: All mastectomy patients benefit from our comprehensive program — including lymphedema risk management, radiation fibrosis treatment, and the Strength After Breast Cancer exercise program. See our full program →

Post-Mastectomy Concerns We Treat

  • Arm and shoulder stiffness and restricted range of motion
  • Axillary web syndrome (cording) — firm cord-like bands restricting arm elevation
  • Chest wall tightness and pectoral muscle restriction
  • Mastectomy scar management — preventing adhesion, tightening, and scar shelf
  • Lymphedema risk assessment and early intervention
  • Posture changes from chest wall asymmetry and protective guarding
  • Post-mastectomy pain syndrome (PMPS)
  • Cancer-related fatigue during and after adjuvant treatment
  • Return to function, daily activities, and meaningful occupation

Our Treatment Approach

01

Comprehensive Assessment

We evaluate shoulder and arm range of motion, chest wall mobility, scar status, posture, and lymphatic risk factors. Baseline arm measurements are established. We review your surgical details, axillary node status, radiation history, and reconstruction plans.

02

Arm & Shoulder Mobility Restoration

Progressive, guided shoulder and arm mobility exercises beginning as soon as your surgeon clears movement — typically within 1–2 weeks post-operatively, adapted to your surgical constraints and healing stage.

03

Manual Therapy & Scar Management

Scar desensitization, mobilization, and soft tissue treatment at the mastectomy scar and any reconstruction incisions. Myofascial release for chest wall tightness. Cording treatment when present. See scar management services →

04

MLD & Lymphedema Management

Manual Lymphatic Drainage for post-surgical chest wall and arm swelling. Lymphedema risk monitoring throughout treatment, with early intervention if subclinical changes are detected. See our lymphedema program →

05

Strength & Functional Rehabilitation

Progressive upper body strengthening through our Strength After Breast Cancer (ABC) program — rebuilding strength, endurance, and confidence needed to return to daily life and valued activities.

Frequently Asked Questions

When can I start rehabilitation after total mastectomy?

Most patients can begin gentle arm and shoulder movement within 1–2 weeks post-operatively once drains are managed and your surgeon has cleared movement. We confirm timing with your surgical team.

I'm having immediate reconstruction. How does rehabilitation change?

Reconstruction adds specific movement restrictions that must be respected during rehabilitation. We address both the mastectomy and reconstruction components in an integrated plan.
