Implant-based breast reconstruction is the most common reconstruction method after mastectomy. In two-stage reconstruction, a tissue expander is placed at the time of mastectomy, gradually filled with saline to stretch the chest wall tissue, then exchanged for a permanent implant in a second procedure. Each stage has specific rehabilitation implications: the expansion phase creates progressive chest wall tightness and shoulder elevation restriction; after exchange surgery, the reconstruction site requires additional recovery support and ongoing scar and capsule management.
Direct-to-implant reconstruction: If you had immediate implant placement without a tissue expander, the expansion phase is absent but the chest wall rehabilitation and scar management needs are otherwise similar.
Tissue Expander & Implant Concerns We Treat
- Chest wall tightness and pectoral tension throughout the expansion phase
- Shoulder elevation restriction worsening with each expansion fill
- Chest wall scar management at the mastectomy and drain sites
- Implant settling support after final exchange surgery
- Capsular contracture risk reduction through early mobility and MLD
- Axillary web syndrome from sentinel node or axillary dissection
- Lymphedema risk monitoring throughout the reconstruction process
Our Treatment Approach
Expansion Phase Assessment
We assess chest wall mobility, shoulder range of motion, and reconstruction site status. The rehabilitation plan is structured around your expansion schedule — providing targeted chest wall stretching and shoulder mobility work adapted to progressively increasing tissue tension.
Expansion Phase Shoulder & Chest Wall Work
Gentle, targeted chest wall stretching and shoulder mobility exercises coordinated with your expansion visits. As fill volume increases, we adapt the program continuously.
MLD for Reconstruction Site Swelling
MLD for breast and chest wall swelling during and after reconstruction. Supports the most favorable tissue environment for implant settling and capsule formation.
Exchange Surgery Recovery
After the expander is exchanged for the permanent implant, we address the additional recovery — swelling, any new incision scars, and implant settling — as a distinct rehabilitation phase.
Long-Term Scar & Capsule Management
Ongoing scar mobilization and MLD to support pliable scar tissue and the softest possible capsule formation around the implant.
Tissue expander and implant reconstruction rehabilitation spans multiple surgical stages. Our certified team supports every phase.

Frequently Asked Questions
My expander fills are getting very tight and painful. Can rehabilitation help?
Yes. Targeted chest wall stretching and shoulder mobility work — performed around your fill schedule — significantly reduces the tightness and discomfort that accumulates between and after fills.
How does rehabilitation change after my exchange surgery?
The exchange surgery adds a recovery period from the second procedure — swelling, incision healing, and adaptation as the permanent implant settles. We address this as a distinct recovery phase.
