Radical Mastectomy & Its Extensive Rehabilitation Needs

Radical mastectomy (Halsted mastectomy) removes the entire breast, both pectoralis major and pectoralis minor muscles, and all axillary lymph nodes. While largely replaced by modified radical mastectomy in modern practice, many patients who underwent radical mastectomy in earlier decades continue to live with its long-term functional effects. The removal of both pectoral muscles produces the most significant functional rehabilitation challenge of any mastectomy type: significant chest wall thinning, loss of anterior pushing strength, and the highest lymphedema risk of any breast cancer surgery.

Part of our complete breast rehabilitation program: All mastectomy patients benefit from our comprehensive program — including lymphedema risk management, radiation fibrosis treatment, and the Strength After Breast Cancer exercise program. See our full program →

Post-Radical Mastectomy Concerns We Treat

  • Loss of pectoralis major — anterior pushing strength, arm elevation, and chest wall contour
  • Loss of pectoralis minor — shoulder blade stability
  • Highest lymphedema risk of any mastectomy type — complete axillary clearance
  • Significant shoulder weakness and functional limitation
  • Chest wall asymmetry and pronounced concavity
  • Axillary web syndrome and extensive axillary scar tissue
  • Post-mastectomy pain syndrome from extensive nerve involvement
  • Long-term adaptive postural patterns

Our Treatment Approach

01

Comprehensive Assessment & Goal Setting

Thorough assessment of shoulder strength, range of motion, scapular stability, chest wall mobility, and lymphatic status. Functional goal-focused plan developed addressing both immediate recovery and long-term adaptive challenges.

02

Shoulder & Arm Rehabilitation

Progressive shoulder strengthening and mobility working with the changed muscle anatomy after pectoral removal — identifying compensatory muscles and developing individualized strengthening around the specific anatomy of your surgery.

03

Manual Therapy & Scar Management

Dedicated manual therapy for the chest wall — addressing concavity, adherence, and skin mobility changes. Myofascial release and scar mobilization applied systematically to the chest wall and axilla.

04

Lymphedema Management

Complete axillary clearance creates the highest lymphedema risk of any mastectomy. CLT-certified MLD, compression, and self-management education provided. Long-term monitoring essential. See our lymphedema program →

05

Adaptive Strategies & Functional Return

Occupational therapy for adaptive strategies around the changed arm and shoulder function — returning to daily activities, work, and meaningful occupation within the specific anatomical changes of radical mastectomy.

Radical mastectomy creates extensive, permanent changes. Long-term, expert rehabilitation is essential.

Frequently Asked Questions

I had a radical mastectomy many years ago and my arm is very swollen. Can you help?

Yes. Lymphedema following radical mastectomy responds to treatment regardless of how long it has been present. We provide comprehensive CDT and can significantly improve swelling and quality of life.

Is it too late for rehabilitation of my shoulder after radical mastectomy?

Shoulder rehabilitation can produce meaningful functional improvement at any stage after radical mastectomy. The goal is to optimize function of remaining muscles and develop adaptive strategies that maximize arm and shoulder capability.
