Risk-Reducing Mastectomy & Its Rehabilitation Needs

Prophylactic mastectomy is a risk-reducing surgery performed in individuals with significantly elevated breast cancer risk — most commonly BRCA1 or BRCA2 carriers — reducing lifetime breast cancer risk by approximately 90–95%. Bilateral prophylactic mastectomy (BPM) is almost always combined with immediate bilateral reconstruction. The rehabilitation needs are those of bilateral mastectomy with bilateral reconstruction — simultaneously managing both sides with their specific healing timelines and reconstruction types. The physical rehabilitation needs are essentially identical to those after therapeutic mastectomy; what differs is the emotional context.

Part of our complete breast rehabilitation program: All mastectomy patients benefit from our comprehensive program — including lymphedema risk management, radiation fibrosis treatment, and the Strength After Breast Cancer exercise program. See our full program →

Post-Prophylactic Mastectomy Concerns We Treat

  • Bilateral arm and shoulder mobility restriction
  • Bilateral scar management across all mastectomy and reconstruction incision sites
  • Bilateral tissue expander management where immediate expander placement is performed
  • Bilateral lymphedema risk when sentinel node biopsy is performed on both sides
  • Chest wall tightness and pectoral changes from bilateral surgery
  • Reconstruction settling and symmetry concerns during bilateral healing

Our Treatment Approach

01

Bilateral Assessment

We assess both sides simultaneously — mobility, scar status, tissue expander fill levels, and any asymmetry between left and right healing. Both sides may heal at different rates or have different reconstruction types.

02

Bilateral Arm & Shoulder Rehabilitation

Progressive mobility work addressing both sides — adapting to any differences between sides in surgery type, reconstruction approach, or healing stage.

03

Bilateral Scar Management

Coordinated scar management across all bilateral mastectomy and reconstruction incision sites, addressing both sides systematically.

04

Lymphedema Risk Management

When sentinel node biopsy is performed bilaterally, lymphedema risk exists on both sides. Baseline measurements established and monitoring conducted on both arms. See our lymphedema program →

05

Functional Return & Wellbeing Support

Progressive functional rehabilitation supporting return to daily activities, exercise, and meaningful occupation, honoring both the physical and psychological dimensions of prophylactic mastectomy recovery.

Prophylactic mastectomy physical rehabilitation is as extensive as therapeutic mastectomy. Our certified team provides comprehensive support.

Frequently Asked Questions

I had a prophylactic mastectomy as a BRCA carrier. Do I need the same rehabilitation as someone treated for cancer?

Yes — the physical rehabilitation needs after prophylactic mastectomy are essentially identical to those after therapeutic mastectomy. The surgery and reconstruction are the same. The distinction is in the emotional and psychological context, not the physical recovery requirements.

I'm having bilateral prophylactic mastectomy with immediate implant reconstruction. What does recovery look like?

Bilateral mastectomy with bilateral immediate reconstruction is a significant surgery. Arm and shoulder mobility work begins within 1–2 weeks; both sides are addressed simultaneously; scar management begins once incisions have adequate healing; and chest wall tightness and implant settling are managed over the following weeks and months.
