Why the Abdomen Swells After Birth

Many new mothers are surprised by how long abdominal swelling persists after birth. The uterus takes several weeks to return toward its pre-pregnancy size, and during that time the abdominal wall, surrounding fascia, and overlying tissue continue to carry significantly more fluid than before pregnancy. This is compounded by the total fluid volume that was accumulated during pregnancy now requiring reabsorption and elimination.

For C-section patients, the additional surgical swelling from the operative procedure adds to the picture — and diastasis recti (separation of the midline abdominal muscles) can affect how the abdominal wall functions during this reabsorption process. How is postpartum abdominal swelling different from diastasis recti?

Abdominal swelling is excess fluid in the tissue; diastasis recti is a separation of the abdominal muscles affecting structural integrity. The two commonly co-exist. MLD addresses the fluid component; pelvic floor physiotherapy addresses the diastasis. We assess for both and refer for core rehabilitation when appropriate.

Is it normal for my abdomen to still be very swollen 3 weeks after birth?

Some abdominal fullness is expected in the first 2–4 weeks as the uterus involutes and fluid is reabsorbed. Significant, persistent puffiness that is not clearly improving after 3–4 weeks may benefit from lymphatic support. An evaluation will clarify what is contributing.
