Why Surgery Causes Swelling & How Lymphatic Therapy Helps

Post-operative swelling — also called post-surgical edema — is a universal response to surgical trauma. When tissue is cut, manipulated, or repositioned, local blood vessels become more permeable and fluid leaks into the surrounding interstitial space. Simultaneously, the lymphatic vessels responsible for clearing that fluid are themselves disrupted, reducing the body's ability to drain the accumulating fluid efficiently.

The result is swelling, puffiness, firmness, and sometimes bruising that can persist well beyond the initial days after surgery. While some degree of post-operative swelling is expected and unavoidable, excessive or prolonged edema delays tissue healing, increases discomfort, and can contribute to the formation of fibrotic scar tissue if left unaddressed.

Manual Lymphatic Drainage (MLD), performed by a certified lymphatic therapist, is one of the most effective tools available for reducing post-surgical edema. By gently stimulating the superficial lymphatic network, MLD accelerates the clearance of excess fluid, reduces inflammatory load, and supports faster, cleaner tissue healing. Many surgeons across New York City recommend MLD as a standard component of post-operative recovery for their patients.

MLD is not massage. Post-operative swelling requires very light, specific, directional strokes mapped to the lymphatic anatomy — not the deep pressure of traditional massage, which can be harmful to healing tissue. Symptoms of Post-Operative Swelling We Treat

Manual Lymphatic Drainage

Gentle, rhythmic MLD strokes are applied in a sequence designed to open proximal lymph node basins first, then guide fluid away from the congested surgical area toward functioning drainage pathways. Sessions typically last 45–60 minutes and are adapted to your comfort and response at each visit.

Compression Guidance

Where appropriate, we advise on compression garment use between sessions to sustain the fluid reduction achieved during MLD. Compression type, pressure, and wear schedule are individualized to your surgery and the areas affected. We work within your surgeon's garment recommendations.

How soon after surgery can I start MLD?

Many surgeons recommend beginning MLD within 1–2 weeks post-operatively, once the incisions are sufficiently healed and you have surgical clearance. Some procedures allow for even earlier sessions. We always confirm appropriate timing with your surgical team before beginning treatment.

How many sessions will I need?

This depends on the extent of surgery, the degree of swelling, and how your body responds to treatment. Many patients see meaningful improvement within 3–6 sessions. More extensive procedures or persistent swelling may benefit from a longer course of treatment.

Is MLD covered by insurance for post-surgical swelling?

Coverage varies by plan and clinical indication. When MLD is provided as physical or occupational therapy for medically documented post-surgical edema, it is sometimes covered. We recommend contacting your insurer and are happy to provide supporting documentation.
