Peripheral edema refers to the accumulation of excess fluid in the tissues of the extremities — most commonly the legs, ankles, and feet. It presents as visible swelling, puffiness, and often pitting in the affected tissue. Causes are diverse, and accurate identification of the underlying mechanism is important for effective treatment.
The lymphatic system is involved in many but not all forms of peripheral edema. When lymphatic involvement is present, lymphatic therapy can be highly effective. When primary causes are cardiac, renal, or hepatic, medical management is the priority and lymphatic therapy serves as an important adjunct to reduce edema burden and support tissue health.
Medical evaluation first: New or sudden-onset peripheral edema — particularly when bilateral and accompanied by shortness of breath or chest discomfort — should be evaluated by a physician before beginning lymphatic therapy. We always review your medical history and current medical management before beginning treatment.
Common Causes With Lymphatic Components
Primary or secondary lymphatic insufficiency
Produces chronic lower extremity or arm swelling. The clearest indication for lymphatic therapy and the primary focus of our evaluation.
Combined venous and lymphatic insufficiency
Common in CVI where the lymphatic system has been overwhelmed by chronic venous fluid load. Requires modified MLD technique.
Recurrent swelling without clearly identified systemic cause
Often has a lymphatic component amenable to MLD and compression. Evaluation determines the most appropriate approach.
Several medications produce peripheral edema as a side effect
MLD reduces the fluid burden while the underlying medication is managed with your physician. Several medication classes are associated with this effect.
Prolonged sitting, reduced mobility, or chronic inactivity
Impairs calf muscle pump function and lymphatic return. MLD and movement guidance directly address this mechanism.

Manual Lymphatic Drainage
MLD adapted to the specific cause and distribution of your edema. Drainage routes and technique are modified based on whether venous, cardiac, or renal factors are also present.
Compression Therapy
Appropriate compression — hosiery, bandaging, or combination — is central to managing peripheral edema of lymphatic or mixed origin. We measure, advise on, and fit compression for your specific presentation.

My legs have been swollen for months. Is it too late for lymphatic therapy?
No. Peripheral edema of lymphatic or mixed origin often responds to MLD and compression even when longstanding. An evaluation will clarify the contributing factors and what is achievable.
How do I know if my leg swelling is lymphedema?
Lymphedema and other forms of peripheral edema can look similar but have different underlying mechanisms. A clinical assessment by a CLT-certified therapist — assessing tissue character, distribution, and history — is the most reliable way to differentiate them.
