For premenopausal women with hormone receptor-positive breast cancer, reducing estrogen production through surgical oophorectomy (BSO) or medical ovarian suppression is an important treatment component. Abrupt estrogen deprivation produces a sudden surgical or medically-induced menopause — with joint pain, muscle aching, reduced bone density, and genitourinary changes that are compounded when combined with aromatase inhibitor therapy. These symptoms are well-recognized consequences of estrogen deprivation that respond to physical and occupational therapy.
BRCA carriers: Some BRCA mutation carriers undergo oophorectomy as a risk-reducing procedure. The physical rehabilitation needs — managing surgical menopause effects — are essentially identical regardless of whether the surgery is therapeutic or preventive.
Oophorectomy & Ovarian Suppression Side Effects We Treat
- Joint pain and musculoskeletal symptoms from estrogen deprivation — similar to aromatase inhibitor-associated musculoskeletal syndrome
- Bone density management — weight-bearing exercise and fall prevention to counteract estrogen-deprivation bone loss
- Post-surgical recovery from laparoscopic oophorectomy — abdominal and pelvic recovery
- Fatigue and reduced endurance from hormonal changes
- Balance and fall risk from bone density reduction and joint symptoms
- Return to exercise guidance adapted to surgical menopause physiology
Our Treatment Approach
Surgical & Hormonal Status Assessment
We review whether your ovarian suppression is surgical or medical, your current hormone status, combined aromatase inhibitor use, and your specific musculoskeletal and functional symptoms.
Post-Surgical Recovery (Oophorectomy Patients)
For patients who had laparoscopic BSO, gentle post-surgical rehabilitation addressing abdominal and pelvic recovery and return to activity.
Bone-Loading Exercise Program
Weight-bearing and resistance exercise to counteract the accelerated bone density loss from estrogen deprivation — one of the most evidence-based interventions for maintaining bone health during ovarian suppression.
Joint & Musculoskeletal Management
Exercise therapy and joint mobilization for estrogen-deprivation associated joint pain — using the same evidence-based approach as for aromatase inhibitor joint symptoms.
Pelvic Floor Referral & Coordination
Genitourinary changes from estrogen deprivation frequently benefit from pelvic floor physiotherapy. We provide appropriate referral and coordinate with pelvic floor specialists.
Oophorectomy and ovarian suppression produce significant musculoskeletal changes that rehabilitation can meaningfully address.

Frequently Asked Questions
I had my ovaries removed for BRCA risk reduction and my joints are extremely painful. Is that related?
Yes. Bilateral oophorectomy produces sudden surgical menopause — and estrogen deprivation is strongly associated with joint pain and musculoskeletal symptoms similar to AIMSS. This responds to exercise therapy and joint management.
How important is weight-bearing exercise after oophorectomy?
Very important. Estrogen loss through oophorectomy significantly accelerates bone turnover and density reduction. Weight-bearing and resistance exercise is the most effective non-pharmacological intervention for maintaining bone density and should begin as soon as you are cleared.
