Nipple-Sparing Mastectomy & Its Unique Rehabilitation Needs

Nipple-sparing mastectomy (NSM) preserves both the breast skin envelope and the nipple-areola complex (NAC), removing only the breast tissue beneath. It offers the most natural aesthetic outcome and is typically performed with immediate reconstruction. The preserved NAC undergoes significant sensory changes after NSM — numbness, altered sensation, or hypersensitivity — that evolve throughout the recovery period. Will I have sensation in my nipple after nipple-sparing mastectomy?

Nipple sensation is significantly altered after NSM as the nerve supply is disrupted when glandular tissue is removed. Most patients experience numbness initially, with variable partial recovery over 12–24 months. Sensation-preserving techniques aim to improve this outcome.

My nipple looks slightly darker after NSM — should I be concerned?

Any color changes in the NAC after NSM should be reported to your surgical team promptly, as they may indicate compromised blood supply. Contact your surgeon immediately if you notice changes in nipple color or skin appearance.
