Nipple-sparing mastectomy (NSM) preserves both the breast skin envelope and the nipple-areola complex (NAC), removing only the breast tissue beneath. It offers the most natural aesthetic outcome and is typically performed with immediate reconstruction. The preserved NAC undergoes significant sensory changes after NSM — numbness, altered sensation, or hypersensitivity — that evolve throughout the recovery period. Rehabilitation addresses these sensory changes through systematic desensitization, alongside the arm mobility, axillary management, and lymphedema risk needs common to all mastectomy patients.

Assessment Including NAC Sensory Status
We assess NAC sensation, healing status, and any areas of hypersensitivity. NAC sensory recovery is mapped and monitored throughout rehabilitation.
Nipple-Areola Complex Desensitization
Systematic desensitization using progressively textured materials normalizes sensory processing at the preserved nipple and surrounding skin — performed only when the NAC has demonstrated reliable viability.
Arm & Shoulder Mobility Restoration
Progressive shoulder and arm mobility addressing mastectomy and axillary surgery stiffness, adapted to reconstruction constraints and tissue expander expansion schedule.

Frequently Asked Questions
Will I have sensation in my nipple after nipple-sparing mastectomy?
Nipple sensation is significantly altered after NSM as the nerve supply is disrupted when glandular tissue is removed. Most patients experience numbness initially, with variable partial recovery over 12–24 months. Sensation-preserving techniques aim to improve this outcome.
My nipple looks slightly darker after NSM — should I be concerned?
Any color changes in the NAC after NSM should be reported to your surgical team promptly, as they may indicate compromised blood supply. Contact your surgeon immediately if you notice changes in nipple color or skin appearance.
