Lipolymphedema is a combined condition in which lipedema — a disorder of pathological adipose tissue deposition — has progressed to the point where it secondarily impairs lymphatic drainage, producing a lymphedema component in addition to the underlying lipedema. It typically represents advanced-stage lipedema (Stage 3 or above) where the mechanical burden of abnormal fat tissue overwhelms local lymphatic capacity.
Lipolymphedema presents with the characteristic bilateral, disproportionate fat distribution of lipedema — stopping abruptly at the ankles — combined with pitting, non-resolving swelling that extends to the feet and does not reduce with elevation. This swelling beyond the ankles is the key clinical differentiator from pure lipedema.
Management requires expertise in both conditions simultaneously — a combination that demands specific CLT training and experience. MLD pathways, compression strategies, and self-management education must account for both the lipedema and lymphedema dimensions.
Differentiation matters: Lipolymphedema is frequently misdiagnosed as pure lymphedema, pure lipedema, or obesity. If you are uncertain which condition you have, our CLT-certified therapists can assist with clinical differentiation as part of your evaluation.
Lipolymphedema Symptoms We Treat
- Bilateral leg heaviness, aching, and pain from the lipedema tissue component
- Non-resolving swelling extending to the feet — the secondary lymphedema component
- Easy bruising and tenderness characteristic of lipedema fat
- Skin changes including fibrosis, hardening, and papillomatosis in advanced cases
- Progressive tissue changes impairing mobility and daily function
- Recurrent infection risk from the compromised lymphatic tissue environment
Our Treatment Approach
Integrated Assessment
We assess both lipedema and lymphedema components — staging each, identifying the distribution of abnormal fat tissue versus fluid, and developing a plan addressing both simultaneously.
Adapted Manual Lymphatic Drainage
MLD for lipolymphedema uses modified drainage routes that navigate around obstructive lipedema tissue to access functioning lymphatic pathways. Technique is adapted to the fibrotic tissue environment of advanced lipolymphedema.
Compression Therapy
Compression must address both conditions — typically requiring higher levels than for pure lymphedema alone, with careful attention to garment fit given the specific tissue distribution of lipedema. We measure, advise, and fit compression appropriate to your presentation.
Self-Management Education
Education on self-drainage, skin care, infection prevention, appropriate movement, and long-term management strategies. The condition is chronic and lifelong — equipping patients with effective self-management tools is a priority.
Lipolymphedema requires specialist expertise in both conditions. Our certified Midtown Manhattan team is here to help.

Frequently Asked Questions
How do I know if I have lipolymphedema rather than just lipedema?
The key distinction is swelling that extends to the feet and does not resolve with elevation — this indicates the secondary lymphedema component defining lipolymphedema. In pure lipedema, the feet are typically spared. A CLT-certified therapist can assist with clinical differentiation at your evaluation.
Is treatment for lipolymphedema very different from lymphedema treatment?
Yes — in important ways. The presence of lipedema fat tissue modifies MLD drainage routes, compression strategies, and the overall management approach. Our therapists are specifically trained in the adaptations required.
