How Surgical Scars Form & Why They Need Management

Every plastic surgery procedure produces scars — both at the formal incision lines used to access and reshape tissue, and at the small puncture sites where liposuction cannulas were inserted. How these scars heal is not simply a matter of chance: scar quality is directly influenced by the care and management they receive during the critical healing window, which extends from initial wound closure through approximately 12–18 months post-operatively.

Without management, surgical scars can become raised, thickened, hyperpigmented, or adherent to the underlying tissue — affecting both their appearance and the function of the surrounding tissue. Scars that adhere to deeper structures can restrict movement, create a tethered or dimpled skin appearance, contribute to chronic tightness and discomfort, and transmit tension to surrounding tissue that distorts the surgical result. Effective scar management prevents these outcomes and, where they have already begun to develop, can significantly reverse them.

Cannula insertion scars — the small circular puncture sites from liposuction — are often overlooked in post-surgical care. When can I start scar management after plastic surgery?

Active scar management can begin once the incision is fully closed and there is no open wound — typically 6–8 weeks post-operatively, though this varies by procedure and individual healing. Desensitization can often begin earlier, over intact closed skin. We confirm readiness for each scar at your initial assessment.

Do I need to treat cannula insertion scars or just the main incision?

Both benefit from management. The main incision scar is larger and more visually prominent, but multiple cannula insertion scars can also become raised, pigmented, or irregular without management — and the subcutaneous fibrosis radiating from each entry point is an important contributor to the overall texture changes after liposuction.

My scar is over a year old. Can scar management still help?

Scar remodeling continues for up to 2 years post-surgery. Mature scars of 12–18 months age can still respond to manual mobilization, particularly for adhesion and tissue tethering. Hyperpigmentation and raised texture also respond to ongoing management. We assess each scar individually and provide realistic expectations for what is achievable.
