What Is Post-Surgical Fibrosis?

Fibrosis is the formation of excessive fibrous connective tissue in response to surgical trauma. When tissue is cut, sutured, or manipulated — as in any plastic surgery procedure — the body's healing response lays down collagen to repair the disruption. Under ideal conditions, this collagen matures into pliable, organized scar tissue. Under less-than-ideal conditions — when the inflammatory phase is prolonged, when lymphatic drainage is slow, or when the healing tissue is subjected to repeated micro-trauma — the collagen becomes disorganized, dense, and adherent: fibrosis.

In the context of plastic surgery, fibrosis most commonly presents as areas of firmness, hardness, or lumpiness beneath the skin — particularly in areas where liposuction cannulas have passed, where fluid has pooled, or where tissue flaps have been elevated. It is one of the most common post-operative complaints after body contouring procedures and, without skilled intervention, can significantly compromise the smooth, even aesthetic result surgery was designed to produce.

The good news: fibrosis responds to treatment — especially when addressed early. A combination of Manual Lymphatic Drainage and targeted soft tissue mobilization is the most effective conservative approach for both preventing fibrosis formation and softening established fibrotic tissue.

Prevention is far easier than treatment. Fibrosis that is addressed within the first 6–8 weeks of surgery responds significantly better than established fibrosis that has been present for months. Manual Lymphatic Drainage

MLD reduces the inflammatory and fluid environment that sustains fibrosis formation. By clearing the protein-rich fluid that drives collagen overproduction, MLD interrupts the fibrotic process at its source — most effectively when begun early in the post-operative period.

Soft Tissue Mobilization

Once healing allows, targeted manual techniques are applied directly to fibrotic areas — using cross-friction, myofascial release, and instrument-assisted soft tissue techniques as appropriate to break down dense collagen cross-links and restore tissue pliability. This is combined with MLD, not performed instead of it.

Cupping & Advanced Techniques

Where clinically appropriate, silicone cupping therapy can be applied over fibrotic areas to create targeted decompression and improve tissue glide. I had liposuction 3 months ago and still have hard lumpy areas. Is it too late to treat?

No. While fibrosis that is addressed within the first 6–8 weeks responds most readily, established fibrosis of 3–6 months duration still responds meaningfully to a combination of MLD and soft tissue mobilization. The treatment course may be longer, but significant improvement is achievable. We recommend beginning as soon as possible rather than waiting further.

How many sessions does fibrosis treatment take?

Early fibrosis (within 6–8 weeks of surgery) often responds within 4–8 sessions. Established fibrosis may require a longer course of 10–15 sessions or more, with treatment frequency adapted to your response. We reassess at each visit and adjust the plan accordingly.

Will the hard areas completely go away?

Response varies by the age and density of the fibrosis, the procedures involved, and individual healing characteristics. Many patients achieve complete or near-complete softening of early fibrosis with consistent treatment. Established fibrosis typically achieves significant improvement — meaningful softening and improved texture — though complete resolution may not always be possible.
