Chronic venous insufficiency (CVI) occurs when the one-way valves within the leg veins fail to function adequately, allowing blood to pool in the lower extremities rather than returning efficiently to the heart. The resulting elevated venous pressure forces fluid into the surrounding tissue — producing the characteristic leg swelling, heaviness, aching, and skin changes associated with CVI.
The lymphatic system is intimately connected to venous function: when venous pressure is chronically elevated, the lymphatic system compensates by clearing the excess fluid load. Over time this overloads lymphatic capacity, leading to a secondary lymphatic insufficiency (phlebolymphedema) that compounds the swelling and tissue changes of CVI.
We work in coordination with your vascular physician. MLD and compression are important conservative management tools for CVI that complement — and do not replace — your medical management. We do not treat varicose veins or venous disease directly.

We review your venous disease history, current medical management, and imaging findings. We communicate with your vascular specialist to ensure our approach is coordinated with your overall venous management plan.
Modified MLD for Venous Insufficiency
MLD for CVI uses adapted techniques that account for venous hemodynamic considerations. Drainage routes are selected to reduce venous pressure load. Our therapists are trained in the specific modifications required for safe, effective MLD in the context of venous disease.
Medical-Grade Compression
Appropriate compression class, length, and garment type are guided by your degree of insufficiency and lifestyle. For patients with secondary lymphedema, multi-layer bandaging may be used in an intensive phase before transitioning to maintenance garments.
Movement & Lifestyle Guidance
Calf muscle pump activation — through walking and specific exercises — is one of the most effective supports for venous and lymphatic return. We provide individualized movement guidance, elevation strategies, and skin care advice.

Is MLD safe if I have varicose veins?
In most cases, yes — MLD uses only very light, superficial pressure that does not stress the venous system. However, we review your venous status carefully before beginning treatment. Certain acute venous conditions are contraindicated for MLD and are assessed at your first visit.
Can MLD help with venous ulcers?
MLD and compression therapy improve the tissue environment around venous ulcers by reducing edema and improving lymphatic drainage — both important for wound healing. We coordinate with your wound care team as part of an integrated approach.
