What Is Cannula Line Fibrosis?

Cannula line fibrosis is a specific type of post-liposuction fibrosis that forms directly along the paths traveled by the liposuction cannula during the procedure. As the cannula passes through the subcutaneous fat layer, it creates small tunnels of tissue disruption. The body's healing response fills these tunnels with fibrous collagen — and when this collagen matures in an organized, pliable way, the result is smooth, even tissue. When it matures irregularly — which is particularly likely when lymphatic drainage is impaired or inflammation is prolonged — the result is firm, linear bands or "tracks" of fibrotic tissue running through the treated area.

These bands are often palpable (and sometimes visible) as distinct linear firm zones running in the direction of cannula travel — most commonly in the abdomen, flanks, thighs, and back, where liposuction cannulas cover large distances of tissue. Patients frequently describe feeling "ropes" or "strings" beneath the skin, a board-like linear firmness, or visible ridges that become more apparent with movement or changes in body position.

Cannula line fibrosis is one of the most common complications of liposuction and one of the conditions that responds best to skilled, targeted rehabilitation. With early intervention, the fibrous bands can be significantly softened and in many cases fully resolved — restoring the smooth texture that surgery was intended to produce.

Recognizing This Specific Condition

  • Firm, cord-like bands running in linear paths through treated tissue — often following the direction of cannula passes
  • A "rope under the skin" sensation when palpating the treated area
  • Visible ridges or raised linear areas through the skin, particularly noticeable with movement or in certain lighting
  • Restricted movement or a pulling sensation if bands run across the abdomen, inner thighs, or flanks
  • Worsening firmness in linear patterns as healing progresses past the initial 2–4 weeks

Silicone cupping therapy is applied directly over cannula line fibrosis to create targeted decompression and fascial glide along the bands. How soon after liposuction does cannula line fibrosis develop?

Cannula line fibrosis typically begins forming within the first 2–4 weeks after liposuction, as the initial swelling subsides and the fibrotic changes beneath become palpable. Many patients first notice the bands at 3–5 weeks post-operatively. Beginning treatment at this early stage produces the best outcomes — though established fibrosis of 3–6 months still responds to treatment.

Is cannula line fibrosis the same as a seroma?

No. A seroma is a collection of fluid (lymphatic fluid) in a space created by surgery — it is soft and fluctuant on palpation. Cannula line fibrosis is firm, band-like solid tissue — not fluid. The two can co-exist: prolonged seroma or fluid accumulation creates conditions that favor fibrosis formation, which is why early MLD to prevent seroma indirectly also helps prevent fibrosis.

I can see ridges and lines through my skin after liposuction. Will they go away?

Visible linear ridges after liposuction are often a surface manifestation of cannula line fibrosis in the subcutaneous tissue. With targeted treatment, the underlying fibrotic bands can be softened, which typically improves the surface appearance. Response depends on the age of the fibrosis, tissue depth, and individual healing — earlier treatment consistently produces better visible improvement.
