The Rehabilitation Needs of Brazilian Butt Lift Surgery

Brazilian Butt Lift (BBL) is a body contouring procedure combining liposuction from donor sites (typically abdomen, flanks, waist, and/or thighs) with autologous fat grafting to the buttocks. It is effectively two simultaneous procedures — each producing its own rehabilitation needs — in a single operative session.

The liposuction donor sites require the same lymphatic drainage, fibrosis prevention, and compression management as any liposuction procedure. The fat graft recipient site — the buttocks — has specific positioning and pressure restrictions designed to protect the transferred fat during the critical engraftment period. Rehabilitation must respect these restrictions absolutely while still supporting swelling reduction, lymphatic flow, and tissue quality at the donor sites.

BBL-specific positioning: During the critical fat engraftment period, direct pressure on the buttocks must be avoided. All rehabilitation positioning strictly respects your surgeon's sitting and lying restrictions. We never apply direct pressure to the buttocks graft site during the restricted period.

Can you do MLD on the buttocks after a BBL?

During the critical fat engraftment period — typically the first 6–8 weeks — direct pressure or massage to the buttocks is contraindicated as it can compromise fat graft survival. We focus MLD on the donor liposuction sites and avoid the graft site during this period. Once your surgeon clears direct buttocks contact, we can include that area.

When can I start rehabilitation after my BBL?

We can typically begin at the donor liposuction sites within 1–2 weeks of surgery, respecting all positioning restrictions. We confirm timing and approach with your surgeon before beginning treatment.
